Reporting Forms Printing Instructions.

Print the following documents - all are in pdf format. 1. Covering Letter (2 copies) on A/4 size paper 2. Missing Children reporting Form, print front and back (5 / 10 or more copies) on A/4 size paper.   3. Sample of a filled Missing Children Reporting Form (1 copy) on A/4 size paper. Take all the above to the police station in your locality and on one of the copies of Covering Letter take the acknowledgement and send it to us by snail mail at the address given below. This will help us in knowing to which all police stations the forms have been delivered. Please try to see that the Police Station phone number and email ID (if available) is mentioned.
